How does Linatab M 2.5/500 Tablet work?

LINAPIL M 2.5/500 TABLET is an antidiabetic medicine. It contains two medicines namely metformin and Linagliptin. Metformin makes the body more sensitive to insulin (a hormone involved in glucose metabolism). This helps our body cells to utilize more amount of the circulating blood glucose. Additionally, it can stop glucose production by the liver and reduce its absorption in the body.

Linagliptin controls blood sugar levels by blocking the actions of an enzyme called dipeptidyl peptidase (DPP)-4 that reduces insulin secretion in the body. Collectively, Linatab M 2.5/500 Tablet controls blood sugar levels and improves glycemic control in patients with type II diabetes.

Pharmacology

Linagliptin is used to help people with type 2 diabetes regulate their blood sugar levels. DPP-4 (dipeptidyl peptidase-4) is an enzyme that degrades the incretin hormones GLP-1 (glucagon-like peptide-1) and GIP (glucagon-like peptide-1). Linagliptin is an inhibitor of DPP-4 (dipeptidyl peptidase-4) (glucose-dependent insulinotropic polypeptide). Linagliptin stimulates the release of insulin from pancreatic beta () cells in a glucose-dependent manner while reducing the production of glucagon from pancreatic alpha () cells in the blood, increasing the concentrations of active incretin hormones.

Metformin Hydrochloride is an antihyperglycemic medication of the biguanide class that is used to treat type 2 diabetes. Both baseline and postprandial plasma glucose levels are reduced. It has a different mode of action than sulfonylureas and does not cause hypoglycemia. Metformin Hydrochloride reduces hepatic glucose synthesis, intestinal glucose absorption, and insulin sensitivity by increasing peripheral glucose uptake and utilization.

Dosage & Administration of Linatab M 2.5/500 Tablet

The dosing instructions for Linagliptin & Metformin immediate-release and extended-release tablets are as follows:

Linagliptin & Metformin Immediate-Release Tablet:

  • Dosage should be individualized based on effectiveness and tolerability.
  • The maximum recommended dose is 2.5 mg of Linagliptin and 1000 mg of Metformin Hydrochloride twice daily with meals.
  • For patients not currently treated with Metformin Hydrochloride, initiate treatment with 2.5 mg of Linagliptin and 500 mg of Metformin Hydrochloride twice daily.
  • For patients already treated with Metformin Hydrochloride, start with 2.5 mg of Linagliptin and the current dose of Metformin Hydrochloride twice daily.
  • If patients are already treated with Linagliptin and Metformin Hydrochloride as individual components, they may be switched to this combination with the same doses of each component.

Linagliptin & Metformin Extended-Release Tablet:

  • Dosage should also be individualized based on effectiveness and tolerability, without exceeding the maximum recommended total daily dose of Linagliptin 5 mg and Metformin Hydrochloride 2000 mg.
  • This combination should be taken once daily with a meal.
  • For patients not currently treated with Metformin, initiate treatment with 5 mg of Linagliptin/1000 mg of Metformin Hydrochloride extended-release once daily with a meal.
  • For patients already treated with Metformin, start this combination with a total daily dose of 5 mg of Linagliptin and a similar total daily dose of Metformin once daily with a meal.
  • For patients already treated with Linagliptin and metformin immediate-release tablets, switch to the extended-release tablet containing 5 mg of Linagliptin total daily dose and a similar total daily dose of Metformin once daily with a meal.
  • Follow the specific dosing instructions based on the Linagliptin and Metformin Hydrochloride doses required.

Please note that the extended-release tablet is taken once daily, while the immediate-release tablet may be taken twice daily, as prescribed by your healthcare provider. Always follow your doctor's instructions for the correct dosage and timing of these medications.
