  • Adults and adolescents weighing under 50 kg: 15 mg/kg every 6 hours or 12.5 mg/kg every 4 hours, not exceeding 75 mg/kg per day.
  • Children >2 to 12 years of age: Same dosages as above for weight-based dosing.
Interaction
  • Napa IV may reduce serum levels when taken concomitantly with anticonvulsants such as phenytoin, barbiturates, and carbamazepine. Monitoring of serum levels and adjustment of dosage may be necessary.
  • Prolonged use of Napa IV may enhance the anticoagulant effect of warfarin and other coumarins. Close monitoring of coagulation parameters is advised when using these medications together.
  • Napa IV may have accelerated absorption when taken with metoclopramide and domperidone. Close monitoring for potential adverse effects is recommended.
  • Concomitant use of probenecid may increase serum levels of Napa IV. Dosage adjustment may be necessary to avoid toxicity.
