• Inform your doctor if you experience symptoms such as feeling or being sick, weight loss, pale skin (pallor), or abdominal pain within the first 24 hours as it indicates an overdose.
• Your doctor may regularly monitor your kidney function, liver function, and levels of blood components if you are taking this medicine for long-term treatment.

Use In Special Populations:

• Pediatric Use: Supported by evidence from studies in adults.
• Geriatric Use: No overall differences observed, but caution advised.
• Patients with Hepatic Impairment: Contraindicated in severe impairment; reduced dose may be warranted.
• Patients with Renal Impairment: Longer dosing intervals and reduced total daily dose may be needed in severe impairment.

Pharmacology

• Paracetamol exhibits analgesic action by peripheral blockage of pain impulse generation and antipyresis by inhibiting the hypothalamic heat-regulating center.
• Its weak anti-inflammatory activity is related to inhibition of prostaglandin synthesis in the CNS.
• Paracetamol is thought to act primarily in the CNS by inhibiting cyclooxygenase (COX) enzymes involved in prostaglandin synthesis.
• Unlike NSAIDs, paracetamol does not inhibit COX in peripheral tissues and has no peripheral anti-inflammatory effects.

Dosage & Administration of Napa IV Infusion

The recommended dosages for Napa IV are as follows:

• Adults and adolescents weighing 50 kg and over: 1000 mg every 6 hours or 650 mg every 4 hours, not exceeding 4000 mg per day.
• Adults and adolescents weighing under 50 kg: 15 mg/kg every 6 hours or 12.5 mg/kg every 4 hours, not exceeding 75 mg/kg per day.
• Children >2 to 12 years of age: Same dosages as above for weight-based dosing.

Interaction

• Napa IV may reduce serum levels when taken concomitantly with anticonvulsants such as phenytoin, barbiturates, and carbamazepine. Monitoring of serum levels and adjustment of dosage may be necessary.
• Prolonged use of Napa IV may enhance the anticoagulant effect of warfarin and other coumarins. Close monitoring of coagulation parameters is advised when using these medications together.
• Napa IV may have accelerated absorption when taken with metoclopramide and domperidone. Close monitoring for potential adverse effects is recommended.
• Concomitant use of probenecid may increase serum levels of Napa IV. Dosage adjustment may be necessary to avoid toxicity.
• Napa IV may increase serum levels of chloramphenicol. Monitoring for adverse effects of chloramphenicol is advised when used concomitantly with Napa IV.
• Napa IV absorption may be reduced when administered concomitantly with colestyramine within 1 hour of administration. Consider separating the doses of Napa IV and colestyramine to avoid reduced absorption.
• Concomitant use of Napa IV with phenothiazines may cause severe hypothermia. Monitor patients closely for signs of hypothermia when these medications are used together.

Contraindications

• Known hypersensitivity to paracetamol or any excipients in the intravenous formulation.
• Severe hepatic impairment or severe active liver disease.

Pregnancy & Lactation

• Pregnancy Category C. Use only if clearly needed and after careful benefit-risk assessment.
• Paracetamol is secreted in human milk in small quantities after oral administration.

Precaution & Warnings:

• Hepatic injury, including severe hepatotoxicity and death, may occur with doses higher than recommended.
• Caution in patients with hepatic impairment, alcoholism, chronic malnutrition, severe hypovolemia, or severe renal impairment.
• Discontinue immediately if symptoms of allergy or hypersensitivity occur.

Storage Conditions:

• Store in a cool, dry place away from children.
• For single use only.
• Use within 6 hours after opening. Do not refrigerate or freeze.
