Indication of Adiponil 120mg Capsule

Orlistat is indicated for managing obesity, aiding in weight loss, and maintaining weight when used alongside a reduced-calorie diet. It's also prescribed to reduce the risk of weight regain after prior weight loss. Orlistat 120 mg is recommended for obese patients with an initial BMI ≥30 kg/m2 or ≥27 kg/m2 in the presence of other risk factors like hypertension, diabetes, or dyslipidemia. Orlistat 60 mg is suitable for overweight and obese patients with an initial BMI ≥25 kg/m2.

Pharmacology

Orlistat acts as a reversible inhibitor of gastrointestinal lipase, working in the stomach and small intestine. It forms a covalent bond with the active serine residue site of gastric and pancreatic lipase, rendering them unavailable to hydrolyze dietary fat into absorbable free fatty acids and monoglycerides. The resulting caloric deficit aids in weight control.

Dosage & Administration of Adiponil 120mg Capsule

The recommended dose is one 60 mg or 120 mg capsule taken three times daily with each main meal containing fat, either during the meal or up to 1 hour after. Patients should follow a nutritionally balanced, reduced-calorie diet with approximately 30% of calories from fat. Fat, carbohydrate, and protein intake should be distributed over three main meals. If a meal is occasionally missed or contains no fat, the dose can be omitted. Orlistat may reduce the absorption of fat-soluble vitamins and beta-carotene, so patients should take a multivitamin containing fat-soluble vitamins at least 2 hours before or after Orlistat administration.

Interaction

Orlistat may reduce the absorption of iodine salts and/or levothyroxine, fat-soluble vitamins (A, D, E, K), and beta-carotene. It may decrease plasma levels of ciclosporin and amiodarone. Additionally, it may decrease the efficacy of antiepileptic drugs, antiretrovirals, antidepressants, and antipsychotics. Orlistat can also enhance the anticoagulant effect of warfarin and cause hormonal contraceptive failure.

Contraindications

Orlistat is contraindicated in pregnancy, chronic malabsorption syndrome, cholestasis, and known hypersensitivity to Orlistat or any component of the product.
